Your doctor will need to call with a referral to the Cancer Treatment Center physicians.


Most insurance plans require that you be referred to us for an appointment so it is important your doctor call to refer you for a visit. This way we can be sure to have all of your medical records that we need for your first visit. We may need you to come in prior to your initial visit in order to have release of information forms signed for any records that would provide information regarding your current and past medical history related to your condition to be transferred over.
